FOR THE SPRING PLAYDAY
You will need the following forms:

• Playday Entry Form
• Club/UC Davis Liability Waiver
• Health Information Waiver
• Helmet Waiver (if you choose)
  

Below are the forms for the club.  The Club and Health waivers are required for participation in any rodeo club event.  The Helmet waiver is required for participation in any horseback club event.  The Other Facility waiver is for any other privately owned facility we hold a rodeo event at.
